Sage Care Management serves individuals with disabilities (ages 18+) and their families by creating a tailored, holistic plan of care to address client needs and bring peace to what can be a fraught situation. This personalized approach can include but is not limited to: advocacy; future planning; housing and employment search; ensuring continuity of care among providers; connection to benefits; referrals to resources and supports; navigating complex systems; helping with family dynamics and mediation; and accompanying your loved one to appointments. Together we can overcome barriers and move from survival to a place of thriving and best quality of life for the whole family.

about me
Hello, I'm Jenn. I received a MA from Gordon-Conwell Theological Seminary with the goal of providing a voice for the voiceless and advocating for people with disabilities. I have worked in the field of social services in various capacities since the late 1990s, always focusing on advocating for people in the margins. I take a person-centered and holistic approach to my work, considering the whole person and his/her wishes as a plan of care is developed. My mission and passion is to walk alongside those I serve, whatever barriers they face, empowering, educating and equipping them to overcome life challenges and flourish. My extensive experience with family and friends with disabilities has opened my eyes to the complexity of providing the best care for loved ones facing the challenges disability brings. As an objective consultant, I am honored to help individuals with disabilities reach their greatest potential and live life to the full.
